Telecare in support of Parkinson's, dementia and non self-sufficiency
Fasi offers its clients three innovative telemedicine programmes to support families in their daily management of the disease. Each client has a Care Manager available with expertise in the disease (a nurse or occupational therapist) who can be contacted by phone or video-call, without an appointment, every weekday from 9 a.m. to 6 p.m.
The Care Manager works with the support of a multidisciplinary team comprising: A psychologist, an occupational therapist, a geriatrician and a movement disorder neurologist (who coordinates the team)
Through a structured, multi-dimensional case history, the team identifies the client’s main clinical and care needs and prepares an Individual Care Plan aimed at improved symptom control, the maintenance of functional capacities and the psycho-social well-being of the whole family.
All three telemedicine programmes combine digital technologies and the expertise of experienced healthcare professionals to give our patients access to the most appropriate and effective non-pharmacological interventions in order to:
- manage the typical symptoms of Parkinson’s disease and its impact on functional autonomy;
- monitor factors relating to the person’s frailty and general health;
- prevent and manage behavioural disorders in people with dementia, alleviating stress to caregivers;
- enhance the residual abilities of people with dementia or non self-sufficiency to help them maintain as much autonomy and active participation as possible;
- assist caregivers in enacting strategies to simplify care and reduce fatigue;
- provide emotional support for the well-being of patients and caregivers.
The specialist doctors treating our clients will continue to take care of patient therapy, while the multidisciplinary team will support them with nursing, occupational and behavioural strategies to improve their autonomy and their experience with the disease.
