ParkinsonCare

ParkinsonCare is an innovative, specialist telehealth and telecare service for people with Parkinson’s disease and the symptoms of Parkinsonism, supporting them and their loved ones in their daily lives with the disease.

Each client has a Nursing Care Manager available with expertise in the disease who can be contacted by phone, without an appointment, every weekday from 9 a.m. to 6 p.m.

The Care Manager works with the support of a multidisciplinary team comprising:

A psychologist

An occupational therapist

A geriatrician

A movement disorder neurologist (who coordinates the team)

Through a structured, multi-dimensional case history, the team identifies the client’s main clinical and care needs and prepares an Individual Care Plan aimed at improved symptom control, the maintenance of functional capacities and the psycho-social well-being of the whole family.

ParkinsonCare uses digital technologies to give our clients access to the most appropriate and effective non-pharmacological interventions for managing the symptoms of Parkinson’s and their impact on functional autonomy.

The specialist doctors treating our clients continue to take care of them in terms of therapy, while the ParkinsonCare team supports them with nursing, occupational and behavioural strategies to improve their autonomy and their experience with the disease.

For more information, please call 06/955861 from 9 a.m. to 6 p.m. Monday to Friday.
